• A $15 fee will be assessed for any drop-offs or pick-ups outside of these hours without prior approval.
• A 24 hour cancellation notice is required to avoid a one night boarding charge per pet.

VACCINATIONS: The following vaccinations are required to be current prior to boarding: Rabies, Distemper/Parvo, and Bordetella. If vaccines are not current, they will be administered at the owner's expense. If the client has a primary veterinarian other than CKVS, proof of vaccinations will need to be submitted prior to the boarding reservation. If proof of vaccinations is not submitted by the client/owner prior to, or at the time of the boarding reservation, CKVS reserves the right to administer all vaccinations at the owner’s expense.

INTESTINAL PARASITES: All pets are required to have a negative fecal test for intestinal parasites within 6 months of boarding. Pets that are given a monthly heartworm preventative that contains a comprehensive dewormer are exempt. Fecal results from another veterinary clinic must be provided at the time of boarding. If not current on intestinal parasite control or no current negative fecal test, pet will be tested in the clinic and dewormed if necessary, at the owner's expense.

FLEA CONTROL: Any pet that is admitted for boarding that is infested with fleas and/or ticks will be treated at the owner's expense.

CONSENT FOR TREATMENT: In cases of illness or injury, I, the undersigned, do hereby give my consent for the veterinarians of Central Kentucky Veterinary Services to treat, prescribe for, or operate on my pet(s) while they are boarded at Central Kentucky Veterinary Services. I am responsible for all fees incurred by such services.

CKVS and it's employees will use all reasonable precautions against illness, injury, or escape of my pet, but will not be held liable or responsible in any manner, or circumstances, on account of care, treatment, or safekeeping of my pet, as it is thoroughly understood that I assume all risks.

Should circumstances arise that my pet remains unclaimed after the date, which I have stated as the pick-up date, I understand that written notice will be mailed to the address above. Seven days post written notice, the pet(s) will be considered abandoned and may be rehomed, or taken to a humane society, as Central Kentucky Veterinary Services deems best. It is further understood that such action will not relieve me from paying all costs of service and use of the hospital, including all costs of boarding.

PRICING:

-Large dogs (> 40lbs): $32/night

-Medium dogs (13lbs-40lbs): $28/night

-Small dogs (< 13lbs): $24/night

*A $30 damage fee will be applied and paid for by owner if any damage to CKVS property occurs by boarder*

I hereby consent to and understand all boarding policies, pricing, and options given at Central Kentucky Veterinary Services as stated in the Boarding Release Form.
